Family Health Assessment Questions and Answers: I interviewed a family of four, husband and wife with two kids, ages nine and twelve. 1.) Values, health perception: a.) How do you perceive your health and wellbeing? b.) Is your family considered healthy? c.) Do you guys have any habits that can be detrimental to your health? a) Overall I see myself as being healthy. I do drink alcohol at least three times a week. b) I would consider my family to be healthy; although my husband does smoke. c) My husband is a smoker, and we are trying to help him stop. 2.) Nutrition: a.) How important is nutrition to you? b.) How important is nutrition to your family? c.) Do you feel like you guys get a healthy meal at school? a.) Nutrition is important to me, but I have to admit that I don’t eat as healthy as I should. With our busy schedule, I find myself picking something up on the way home. b.) I feel like my family think that nutrition is important, but they look to me to make the healthy descion because I’m wife and mom. c.) At school we eat a lot of hotdogs and chicken nuggets. We also choose a veggie and a fruit. We eat a lot of the same things every week, it’s like the menu is the same. 3.) Sleep/Rest: a.) How is your family sleeping pattern? b.) How many hours of sleep do you get at night? c.) Does anyone in the family have any sleeping disorders? a.) I think that we all sleep pretty well. The kids have a bedtime of 9:30. I sometimes find myself waking up at least three times a week for like 30 minutes then I’ll fall back to sleep. b.) I get about 6 hours of sleep on average, as well as my husband. c.) My husband does have sleep apnea. He uses a c-pap machine. 4.) Elimination: a.) Do you feel that your urinary out put is sufficient? b.) Does anyone suffer from constipation? c.) Does anyone suffer from Urinary Tract Infections? a.) No problems with anyone going to the rest room. b.) Constipation every now and then. My husband has to take stool softeners 1-2 times a week. c.) I did have a UTI years ago. 5.) Activity/Exercise: a.) How many times a week do you exercise? b.) Would you consider your family to be an active one? c.) Do you guys belong to a gym? a.) I try to work out at least 5 times a week. b.) I consider us to be active. I go to the gym, my husband likes to ride his bike, and my kids are active in sports in school. c.) I belong to 24hr. fitness. 6.) Cognitive: a.) Does anyone in the family have trouble with hearing or seeing? b.) Does anyone have trouble completing his or her activities of daily living? c.) Do you feel that you comprehend and use information correctly? a.) No one with hearing problems. My daughter and I wear glasses. b.) Everyone is health and able to complete their ADLs c.) For the most part I feel that we all are able to comprehend and process information correctly. I know for me if I don’t understand something I have no problem asking questions. 7.) Sensory-Perception: a.) Are you a good listener? b.) How do you stay organized? c.) Do you like to observe people, before engaging in conversation? a.) I think that I’m a good listener. My husband on the other hand has selective hearing in my opinion. b.) The way that I stay organized is by writing things down. I feel lost if I don’t write things down that needs to be done that day. I know my kids like to set reminders on their cell phones. My husband pretty much goes with the flow, and that’s the reason he would have to call me and ask me things over and over again. c.) I feel that I am an observer. When I’m first introduced to someone I like to sit back and observe him or her to see what he or she are all about. My husband on the other hand is a people –person and he can and will talk to any and everyone. 8.) Self –Perception: a.) How do you perceive yourself? b.) How do others perceive you? c.) How do you feel that others perceive your family? a.) I see my self as independent, and a go-getter. I like to get things done right away. My husband is the total opposite; he waits until the last minute to complete tasks. b.) I think that people see me as a leader, then theirs others that probably see me as “bossy”. c.) I would say that people probably see my family as privileged. I’m a nurse and my husband is an Engineer, so people would see us as being accomplished. 9.) Role/Relationship: a.) What is your role in your family? b.) Are you head of household? c.) What role did you play growing up as a child? a.) My role is the consoler, the supporter, the mother and the wife. The overall go to person. My husband is Mr. fix it, a typical man. b.) My husband is head of household c.) Growing up I believe my role was the responsible one. I’m the oldest of three. I felt like I was the one my siblings looked up to, so I had to set a good example. As far as my children they both are always looking out for each other, and they are very close. 10.) Sexuality: a.) Do you feel that you and your spouse have a healthy sex life? b.) Any sexual concerns or problems? c.) Do you talk to your children about sex? a.) I feel that my husband and I have a healthy sex life. I feel that in order to have a healthy sex life you must also have intimacy and companionship. b.) No problems or concerns. c.) We have spoken to our12 year old about sex. I let my husband have the talk because my oldest is a boy. Not the 9 year old yet. 11.) Coping: a.) How do you cope with stress? b.) Do you have a good support system? c.) How does your kids deal with stress? a.) I like to cope with stress by listening to music, taking a walk or talking to my sister. My husband likes to go fishing to relieve stress or sit outside in the backyard by himself for relaxation. b.) I feel that we all have a good support system. We have each other, or other family members. We have a very close family, so if and when we need help someone will always be there. We also have our church family. c.) I honestly think that the children these days use social media to deal with their frustrations. Also we let our kids know that we are always there for them.
