Helpful Information to create Concept Map
MSN Program Outcome #2
Week 4 content topic: Applying Evidence at the Population Level
Week 4 Summary:
This week there was a web site exploration activity involving the IHI, Campaign for Action and IHI Triple Aim. These web sites presented information regarding population health outcomes and health care economics. Inter-professional collaboration was addressed in the Campaign for Action site.
Week 5 content topic: Vulnerable Populations: Culture, Socioeconomics and Race
Week 5 Summary:
In Week 5 you completed your Infectious Disease paper. Are you able to define and apply the epidemiological triad and the HP2020 social determinants of health? The determinants of health, from social to environmental from policy to individual behavior are critical elements to consider related to population health and the role of the NP. Also, in Week 5, the Open Forum gave you opportunity to review culture, access to care, and how provider attitudes impact quality of care provision.
MSN Essential VIII
Assignment Topic: Epidemiological Analysis: Chronic Health Problem
Week 6 Summary:
In Week 6 you submitted your Evaluation of an Epidemiological Problem or Condition Paper. In this paper you analyzed a health problem using a population health focus while integrating epidemiological data and information. For instance, who are the at-risk sub-populations impacted by the chosen problem, the aggregate populations.
Also in Week 6 genetics was the topic of the readings. New terms such as genomics and genetic epidemiology were introduced. The topic of screening and risk assessment using a genetics population health framework was discussed. The article by Belsky, Moffitt, and Caspi (2013) spoke about risk assessment, interventions and environmental causation. This article introduces us to genetic risk scores and family history as a way to assess genetic risks that may be present.
Week 2 content topic: Epidemiological Methods
Through this post the student will discriminate among various screening tools that may be used in the provision of care as an Advanced Practice Nurse (Course Outcome 4)
Week 2 Summary:
In conclusion, the control of infectious disease presents a challenge to the APN on many fronts. The ability to provide effective population-based interventions, in addition to fulfilling legal obligations, can have a profound positive impact on the nation’s health.
Screening/diagnostic tools are often created for population specific use; for instance, gender, age, culture.
NONPF Competencies #4
Exercise 2.6
APRNs should not only recognize but also make it part of their practice to develop strategies to reduce or eliminate health disparities. Review information from Healthy People 2020 and the CDC Office of Minority Health and Health Disparities websites.
What health disparities can you find that are relevant to your community?
How can you better advocate for minority groups who have poorer health outcomes?
What specific objectives in Healthy People 2020 can help this effort?
Introduction: Analysis of the communicable disease (causes, symptoms, mode of transmission, complications, treatment) to include demographic break down that includes age, gender, race, or other at-risk indicators (data per demographics should include mortality, morbidity, incidence, and prevalence).
Determinants of Health: Define, identify and synthesize the determinants of health as related to the development of the infection. Utilize HP2020.
Epidemiological Triad: Identify and describe all elements of the epidemiological triad: Host factors, agent factors (presence or absence), and environmental factors. Utilize the demographic break down to further describe the triad.
Role of the NP: Succinctly define the role of the nurse practitioner according to a national nurse practitioner organization (National Board of Nursing or AANP, for example) and synthesize the role to the management of infectious diseases (surveillance, primary/secondary/tertiary interventions, reporting, data collecting, data analysis, and follow-up). This includes the integration of a model of practice which supports the implementation of an evidence-based practice. Refer to your course textbook for models of practice examples.
